This process involves the transdermal delivery of drugs using ultrasound energy. Phonophoresis was first used by physical therapists, who massaged glucocorticoids and/or local anesthetic creams into the skin with an ultrasound transducer head similar to that used in diagnostic ultrasound. Because ultrasound disrupts the lipid bilayer of the stratum corneum, it has also been explored for the true transdermal application of drugs. Low-frequency sonophoresis, which causes cavitation, or the production of extremely small bubbles in the skin that reportedly form small hydrophilic channels through the stratum corneum, has shown the greatest effects of increasing skin permeability.19 Ultrasound energy on the order of 1 MHz and 0.5 to 1.0 W/cm2 is traditionally used.1,9
Although routinely used by physical therapists, ultrasound is not yet available for home use. In the future, however, it may prove to be an acceptable means of delivering drugs transdermally. Sontra Medical is developing a technology known as SonoPrep for the transdermal delivery of large molecules, including peptides. The manufacturer claims that low frequency ultrasound can provide stable transport for 12 hours following a brief application of 15 seconds or less. For example, according to the company, an insulin patch could be applied for up to 12 hours after a brief ultrasound application.20
As with iontophoresis, phonophoresis may also be used in reverse. Reverse ultrasound technology has allowed for the extraction of interstitial fluid samples for analysis.21 Sontra Medical is developing the Symphony Diabetes Management System, an ultrasound-based continuous glucose monitoring system.22
When an energy source is applied to the skin, some of it is converted to kinetic energy that is exerted over the molecules in the area, resulting in local heating. This temperature rise may account for some of the increased drug permeability seen following patch application through either phonophoresis or iontophoresis.
